N.B.! (i) If you live in another country of the European Union*, Iceland, Liechtenstein, Norway or Switzerland, and - you wish to travel to another country in the European Union*, Iceland, Liechtenstein, Norway or Switzerland to receive planned medical treatment subject to the conditions of Regulations (EC) 883/2004 en 987/2009* (= reimbursement according to the rules and rates of the public health insurance regime of the country where you are to receive medical care), then you must submit your application for prior authorisation* (S2 form*) to the health insurance fund* in your home country, which will send the application on to your Belgian health insurance fund*. Ultimately, it is up to your Belgian health insurance fund* to decide whether or not you are granted prior authorisation* (S2 form*); or - if you wish to travel to another country in the European Union*, Iceland, Liechtenstein, Norway or Switzerland to receive planned healthcare - which does require prior authorisation* – subject to the conditions of the European directive 2011/24/EU* (= reimbursement according to the rules and rates of Belgian compulsory health insurance), then you can submit your application for prior authorisation* (‘ad hoc’ document*) directly to your Belgian health insurance fund*. You must state clearly that your health insurance fund* should deal with this application according to the principles set out in the European directive 2011/24/EU*. (ii) If you live in Cyprus*, Finland, Ireland, the Netherlands, Portugal, Spain, the United Kingdom, Sweden or Norway, and - you currently receive a Belgian pension or Belgian disability benefit, and if you, and your family members, are therefore entitled to healthcare in that country, to be paid for by Belgium, or - you, as a family member, are entitled to healthcare in that country to be paid for by Belgium, while the main beneficiary lives in another country of the European Union*, Iceland, Liechtenstein, Norway or Switzerland, then you must in all cases apply for prior authorisation* to the health insurance fund* in your home country if you wish to travel to receive planned healthcare in another country of the European Union*, Iceland, Liechtenstein, Norway or Switzerland. This is true for both reimbursement procedures. The health insurance fund* in your home country will decide whether you need prior authorisation*, and, if so, whether you are granted it.
